PATIENT GUIDE / 03.1

Arrive with your questions ready.

Use this local checklist to organize practical preparation. A real practice would supply its own approved instructions.

01

Before you arrive

Ask what records the practice needs, how long to allow and whether another appointment is needed for family members.

02

At your first visit

Talk through your questions, the visit format and how the clinician will explain any proposed next steps.

03

Before you decide

Ask for a written explanation of options, expected fees and what your insurer needs to review.

Questions for an actual office

What to confirm with a real practice

Preparation can make a first appointment easier to understand, but each office has its own intake and care process. These are talking points, not instructions or appointments issued by Daymark.

Before check-in

Confirm your arrival time, how the real office supplies secure medical-history forms and whether anything needs to be completed in advance.

During the visit

Bring your questions and ask what an examination, imaging or discussion of possible next steps may involve.

Before leaving

Ask about written instructions, referrals, follow-up questions, expected fees and who to contact at the actual practice.

If you need accommodations, language assistance or a support person, ask the real office what it can provide. The interactive checklist below stays local to this demonstration.
